Lung fibrosis or pulmonary fibrosis is a chronic lung disease where tissue deep in the lungs become thick, stiff and scarred. This permanent damage makes it hard to breathe and reduces oxygen flow into the blood. This permanent scarring makes lungs less elastic, prevents proper expansion and blocks oxygen from passing easily into the bloodstream , leading to severe shortness of breathe and decreased lung capacity.
TYPES OF PLUMONARY FIBROSIS
1. IDOPATHIC PLUMONARY FIBROSIS
Idiopathic pulmonary fibrosis is a specific type of progressive pulmonary fibrosis where the underlying cause is completely unknown .it most commonly affects middle-aged and older adults.
2. FAMILIAL PLUMONARY FIBROSIS
Familial pulmonary fibrosis (FPF) occurs when two or more blood relatives in a family have idiopathic pulmonary fibrosis or another form of fibrotic interstitial lung disease. It accounts for roughly one in five pulmonary fibrosis cases and is commonly linked to genetic factors like telomere-related or surfactant protein gene variant.
CAUSES OF PULMONARY FIBROSIS
1) Autoimmune diseases
Autoimmune diseases cause pulmonary fibrosis when the body’s immune system mistakently attacks its own healthy tissues, including the lungs. This attack triggers chronic inflammation . as the lung tissue tries to repair this ongoing injury, abnormal scarring develops, making the lungs stiff and hard to breathe.
2) Occupational and environmental exposures
Occupational and environmental exposure cause pulmonary fibrosis by repeatedly damaging lung tissues, leading to permanent scar tissue. High risk triggers include inhaling mineral dust like silica and asbestos, organic agricultural dust ,metal and wood dust, and chemical pollutants over long periods .
3) Radiation therapy
Radiation therapy to the chest can cause radiation-induced pulmonary fibrosis , a condition where healthy lung tissue becomes scarred and stiff. This happen when ionizing radiation damages lung cells , triggering an overactive healing response that deposits excess collagen in the treated area.
4) Certain medications
Certain medications can cause drug-induced pulmonary fibrosis by triggering inflammation and tissue scarring in the lungs. Well –known culprits include specific chemotherapy drugs, heart rhythm medications , anti-inflammatory and immunosuppressive agents, and certain common antibiotics .
5) Genetic factors
Genetic factors contribute to about 20% of pulmonary fibrosis cases, known as familial pulmonary fibrosis . Key genetic causes involve mutations in telomere maintenance genes , surfactant protein genes, and the MUC5B promoter variant. SYMPTOMS OF LUNG FIBROSIS
1) Perisistent dry cough
A Persistent dry cough is a hallmark symptom of lung fibrosis ,typically characterized as a stubborn, hacking and non-productive tickle that does not respond to standard cough syrups, antibiotics or common cold medications.
2) Shortness of Breathe
Shortness of Breath is a primary hallmark of pulmonary fibrosis . it is typically starts during physical exertion ,like climbing stairs, but as lung tissue scars and stiffness over time, breathlessness advances to occur during routine daily tasks and eventually even at rest.
3) Fatigue
Fatigue in lung fibrosis is a profound, persistent exhaustion caused by low blood oxygen levels , the hard physical work of breathing , and poor sleep. It is a real physical symptom ,affecting upto 70% of patients and does not improve with simple rest .
4) Chest discomfort
Chest discomfort, tightness or dull ache is a recogonized symptom of lung fibrosis . it typically result from the increased physical effort required to breathe through stiffened lung tissue, muscle strain or inflammation . the pain or tightness can increase when taking deep breaths ,coughing or exercising .
TREATMENT OPTIONS
1) MEDICATIONS
Medications for lung fibrosis cannot reverse existing scar tissue, but specific antifibrotic drugs can slow down the rate of lung function decline and worsening of the disease.
2) OXYGEN THERAPY
Oxygen treatment is a key treatment for lung fibrosis. It corrects low blood oxygen caused by scared lung tissue, eases shortness of breathe , reduces organ strain and helps people stay active . patients usually aim to keep oxygen saturation above 90 % during rest, sleep or exercise.
3) PLUMONARY REHABLITATION
Pulmonary Rehabiltation for lung fibrosis is a supervised medical program combining tailored exercise, breathing techniques and education . it helps patients manage shortness of breathe , increase daily endurance and improve overall physical and mental quality of life.
4) LUNG TRANSPLANTATION
Lung transplantation is a life extending treatment for advanced lung fibrosis, replacing scarred tissue with healthy donor lungs when medications fail. It improves breathing and survival though it requires a rigorous evaluation , lifelong immunosuppression and management of donor shortage.
